Phase I: WHILE YOU ARE AWAY FROM HOME - Homefront Plan & Medical Plan
The goal is to develop plans that will identify information pertaining to your daily life. The planner identifies important contacts, and assigns individual responsibilities to the person(s) designated to act on your behalf in case of emergency. By selectively combining the forms in Phase I to fit your specific needs, you will have a plan for providing information necessary for designated individuals to take over when you are not able. Specifics such as child and pet care decisions, listings of house services, identifying person's with keys, along with all the things necessary to run your daily life. The Homefront Plan you assemble will also be useful while on vacation, routine travel, and extended stay away from home.
The Homefront Plan documents information pertaining to:
· Travel plans
· Home emergency recovery information
· Medical information
· Child care instructions
· Pet care instructions
· Livestock care instructions
In similar fashion, selected use of the forms grouped together will create a Medical Plan. The Medical Plan will contain information about insurance, current and historical health conditions, prescriptions, and a list of emergency contacts, specific direction on power of authority and life saving tactics to be applied.
The goal of the Medical Plan is to build a portable information packet. Keep a copy in your car, take it with you when you travel and provide a copy to the person(s) authorized to act in your behalf should you be unable to provide the information yourself. By selectively combining these forms you will have a plan for providing the medical information necessary for annual records update with current doctors, registering with a new doctor, entering the hospital, or in an emergency healthcare situation.
The Medical Plan provides information pertaining to:
· Health insurance and medical history
· Living Will and other legal documents
· Physicians and prescriptions
· Child emergency medical consent form
